I understand and acknowledge that physical activities may carry some level of inherent risk and that my participation in Forged Men's Ministries and is entirely voluntary.
I acknowledge and assume the risks and hazards associated with physical activities and exercise, including but not limited to injuries, accidents, illness, or death, and I voluntarily participate with knowledge of these risks.
I hereby release and hold harmless, Forged Men's Ministries and , its officers, employees, and volunteers from all claims and causes of action by reason of any injury which may be sustained as a result of this activity, whether on the premises or on the way to or from, or while at the event.
I understand and agree that the Released Parties are not responsible for any personal property that may be lost, damaged, or stolen while participating in Forged Men's Ministries and .
I have carefully read this waiver and release and fully understand its contents. I am aware that by signing this document, I am waiving certain legal rights, including the right to sue. I sign this waiver voluntarily and of my own free will.
